STAR Program Mission. The mission of the Special Treatment and Research (STAR) Program is to achieve health equity through quality care, education, research and community empowerment while creating a culture of continuous improvement, integrity and respect for patients and employees. Acceptance into SUNY Downstate is the goal of many ambitious college students, and its Early Medical Education (EME) Program provides an opportunity for students to learn the ropes and lock down strong study habits before taking on the full course load.
Teaching & Research Center Nurse III
Duties: Provide direct patient care involving the administration and oversight of medications, vaccinations and nursing care assessments. Participate in the development and implementation of teaching plan for patient, family and/or caregivers. Provide telephone and walk in triage for acute patient problems and make appropriate provider referrals. Provide nursing coverage for Monday and Wednesday evening clinic, as needed. Work closely with the STAR Health Center (SHC) Operations Manager and Medical Director to develop, maintain and revise nursing policies and procedures. Serve as team leader and role model for professional and paraprofessional nursing staff, ensure execution and adherence of SHC nursing policies and procedures. Identify substance abuse problems and report issues to the patient’s health care team, including the primary clinician, case manager and care coordinator (if applicable). Promote patient self-care activities and treatments that facilitate optimal levels of functioning. HIV serostatus data tracking and follow up. Tracking and follow up of Buprenorphine patients; participate in the evaluation and screening for Buprenorphine patients. Assist in all clinical procedures including colposcopy, pap, and high resolution anoscopy (HRA) as needed. Tracking and follow-up of HRA patients and procedure clinic. Document all plans and treatment in the medical record; completion of nursing related documentation for NYS DOH and other regulatory agencies. Participate in the development and implementation of multi-disciplinary case managed care, such as attending team huddles, transition and prenatal meetings as needed. Participate in quality improvement activities related to the STAR Health Center, including quality improvement data tracking, including development of PDSA cycles, LEAN, pap screening and PCMH quality improvement indicators. Provide direction to SHC outreach activities including provision of HIV pre and post test counseling, as needed and point of care testing. Assist in administrative responsibilities as needed. Assist in coordinating services for new and emerging SHC programs. RequiredQualifications: RN, NYS License. BS in nursing from accredited nursing school with at least 3 years of ambulatory care experience. Effective written and verbal skills, proficient use of Microsoft office, EMR, and clinic/hospital-based applications. Cultural competency required. Preferred Qualifications: Master’s degree in nursing, public health or related field. HIV/AIDS treatment experience. Substance abuse treatment experience. Experience in academic medical center with integrated care/research. Experience with regulatory requirements of Patient Centered Medical Home.Hours: M-F (37.5 hrs/wk) (some evenings required). Behavioral Health Case Manager
Duties: Work with patients, families, peer advocates and mental health providers to develop comprehensive service plans for clients with serious mental illness (SMI), substance use disorders (SUD) or co-occurring disorders (COD). Provide office or home based individual counseling and group counseling for clients regarding chemical dependency, HIV/HCV prevention and trauma. Refer patients for “wrap-around” recovery support services such as housing, child care, vocational, educational services. Coordinate service plans with behavioral health service providers and community based organizations providing services to clients. Participate in community outreach to promote services and encourage engagement in care. Contribute to patients’ care plan development by regular attendance at case conferences, participate in ongoing quality improvement activities. Utilize phone calls, texting and social media to engage and retain clients in care. Administer mental health and substance use screening tools and conduct patient interviews in accordance with SAMHSA protocols. Record session notes and patient follow-up in the electronic health record. Required Qualifications: Bachelor’s degree in social work or related field with 5 years of experience providing counseling or mental health case management in a health care or community mental health care setting. LMSW may be substituted for two years of experience. Experience conducting psycho-social assessments, intakes and treatment plans, individual, family & group counseling using solution-focused or cognitive behavioral treatment models. Experience working with clients with serious mental illness (SMI) and co-occurring SMI and substance use disorders (SUD). Must be proficient with Office Suite. Must be able to work flexible schedule including some evenings and occasional weekends. Preferred Qualifications: Experience working with LGBTQ populations. Bilingual English/Spanish or English/Creole. Experience with an electronic health record. Hours: Full-time, M-F, 9am-5pm, some evenings & weekends. Salary Range: $48,000-58,000. Part Time Nurse Practitioner/Physician’s Assistant
Duties: Complete medical/physical examination on all patients and provide appropriate documentation in the electronic health record. Order indicated laboratory tests, review all patient and laboratory data and follow-up of all abnormal results as per STAR Health Center policies. Participate in the development and implementation of multidisciplinary case management plans. Refer to sub-specialty medical and surgical evaluations as indicated, and communicate with consultants regarding treatment recommendations. Identify and refer patients for treatment of mental health, substance abuse, domestic abuse/trauma and adherence problems. Provide appropriate interventions in the form of prescription and non-prescription drugs, education, referrals and follow-up care. Provide PrEP/PEP, STI screening and primary care to clients in LGBTQ-specific evening clinic. Participate in STAR Health Center outreach and community education. Function as a teaching professional clinician for STAR Program’s clinical education projects. Participate in quality improvement activities of the STAR Health Center. Adhere to SUNY Downstate’s Principles of Behavior and the STAR Program’s Code of Conduct. Required Qualifications: New York State nurse practitioner or physician assistant license. Preferred Qualifications: Three years’ experience in a primary care setting; two years’ experience in the provision of HIV/HCV care, women’s health and medical care to LGBTQ and/or reentrant populations. Experience in the provision of PrEP/PEP. Bilingual Spanish/English or English Haitian Creole. Hours: Part-time, M-F, 12:00pm to 8:00pm (variable), 20 hours per week. Salary Range: $50,000-60,000. Part Time Research Support Specialist
Duties: Assess barriers to linking patients to care; develop, implement and monitor strategies for overcoming any barriers to linkage and retention; educate, coach and empower patients. Provide HCV rapid counseling and testing. Provide hepatitis c related education, counseling, and patient support. Work closely with HCV team to target, identify, like, and retain clients in program. Provide client follow-up, care coordination, and referrals to ensure comprehensive care delivery. Follow-up with patients and provide appointment reminders via phone or text; provide or refer for appointment accompaniment. Participate in ongoing quality improvement processes; participate on an interdisciplinary team, contribute to patients’ care plan development, and attend case conferences. Required Qualifications: Personal experience with hepatitis C virus required. Must be available to work evening clinics. Effective computer, verbal, organizational, and interpersonal skills. Must be proficient in Microsoft office applications, including Word, Excel, and PowerPoint. Preferred Qualifications: AA/AAS degree in a healthcare related field and at least one year of case management or relevant experience. Experience working with adult HIV positive, HCV positive, substance use and/or high-risk populations preferred. Knowledge of CBOs and training in Motivational Interviewing preferred (may be acquired upon employment). Rapid HCV testing training preferred (will be provided upon employment). Record patient care coordination effort, and patient follow-up in the EHR. Complete encounter forms for entry into database and billing systems as required. Participate in outreach events. Duties:Data entry and management for MACS/WIHS Combined Cohort Study and related substudies. Data management of lab specimen repository. Develop, implement and maintain project related databases and data collection and tracking. Medical record redaction and transmission. Respond to queries for information and generate reports as needed. Required Qualifications: Bachelor’s degree in relevant field. Proficiency using MS Windows, MS Office and Adobe Acrobat Professional. Strong organizational and communication skills. Minimum 1-2 years data entry/management experience. Preferred Qualifications: Master’s degree in relevant field. Experience with LDMS, ArcGIS, RedCap, Qualtrics, SPSS. Medical record abstraction experience. The Exploring Health Careers (EHC) Program is an initiative designed to expose SUNY undergraduate students who are in the Educational Opportunity Program (EOP) to various health professions. Over the course of one 6-week summer session, students will be introduced to related course work for targeted health professions. Students will also participate in workshops, presentations and hands-on learning in clinics at SUNY Downstate Health Sciences University and affiliated sites. Undergraduate Summer Research Program
Applications due March 1, 2018
The Undergraduate Summer Research Program is designed for undergraduate students who have historically been underrepresented in biomedical and health professions. Students will conduct research under the direction of a faculty member. Assignments will be made based upon availability of research opportunities and according to the mutual interests of the faculty sponsors and student participants. Oral presentations are made at the conclusion of the summer.
The program is open to all qualified sophomores and juniors, but it is preferred that students show strong academic promise and come from a background that has in the past denied them opportunities in field of biomedical research.

An initiative to increase the competitiveness of undergraduate pre-medical students, early in their college career. The program is designed to provide medical science lectures and academic support. All of the lectures include a case-based learning approach.
